  •  Amount of taxes that are withheld from the employee’s income
  •   Employee’s year-to-date amount
  • Other deductions (like contributions, life insurance, and other benefits) and other additions to employee’s wages (like compensation, bonus, overtime, etc.)
  • Taxable contributions of the company
  • Employee’s net pay
  • Company name and address
  • An employee social security number (it may be different; every state has its own legal requirements)
